29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE)(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)    The employer is failing to adequately assess the workplace for detection of fall hazards and provide means of protection from the fall hazards that are present.  This was most recently documented on 5/10/2014 at 723 E K Street, McCool Junction, NE. where workers are exposed to fall hazards when on top of tank trailers during loading operations, and are exposed to a fall greater than four feet to the ground below.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:   Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    The employer is failing to protect employees from fall hazards.  The lower and upper platforms in the batching/warehouse area were not equipped with standard railings.    Farmers Cooperative was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1), which was contained in OSHA inspection number 897576, citation number 2, item number 1 and was affirmed as a final order on 8/12/2013, with respect to a workplace located at 2312 State Hwy 41, Wilber, NE 68465.    Farmers Cooperative was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1), which was contained in OSHA inspection number 314061573, citation number 1, item number 2 and was affirmed as a final order on 2/5/12, with respect to a workplace located at 1345 S. Burlington Ave, Exeter, NE 68351.
