On May 15, 2014, and times prior thereto, electrical equipment was not firmly secured to the surface on which it was mounted at the following instances: a. The main air compressor at the main air compressor was not secured to the ground. b. Transformer located on front of the main offices, was not secured to the ground. c. Transformer located near the tool room, was not secured to the ground These conditions exposed employees to electrical shock hazards.
