29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about 16 October 2020, and at times prior thereto, production employees were setting up a printer plate on the Universal Jumbo Die Press Machines without applying an energy isolating lockout device, exposing employees to the hazard of being caught-in the print roller.

On or about 23 October 2020, employees were using a pedestal fan, serial # 005637, that was missing the grounding pin on the power cord plug, exposing the employees to an electrical shock hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 5 October 2020, and at times prior thereto, production employees were feeding foam scrap to the un-guarded in feed rollers on the Foam Repro Line Machine with the machine running, exposing employees to the hazard of being caught-in the rollers. The Austin Foam Plastics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1313724, citation number 1, item number 1 and was affirmed as a final order on 16 August, 2018 , with respect to a workplace located at 2933 A.W. Grimes Blvd., Pflugerville, TX 78660.
