29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: 1015 N. Ridge Ave. bldg., North Press and South Press departments: employer did not provide adequate electrical protective equipment for employees exposed to contact with live parts hazard during line voltage testing on 480V energized electrical control circuits to protect against electrical shock, burns, and Arc flash/blast injuries: a) Rubber insulating gloves with leather protectors. b) Arc-Rated clothing such as arc-rated long-sleeve shirt and pants, or arc-rated coverall. c) Arc-Rated flash suit, hood or arc-rated face shield and arc-rated balaclava. d) Hearing protection (canal inserts). Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c). 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 245 Cortland Ave. Bldg., North, CNC Machining, Super B350 Mach 1070 metal cutting saw: on or about May 16, 2014, employees cutting aluminum and carbon steel raw bar stock into smaller pieces were exposed to contact with rotating blade amputation and laceration hazards because required adequate guarding was not provided. There is no abatement certification or documentation required for this item. The Tella Tool & Mfg. Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 110535, citation number 01, item number 003 and was affirmed as a final order on 5/07/2012, with respect to a workplace located at 1015 N. Ridge Ave., Lombard, IL 60148.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. 1015 N. Ridge Ave. Bldg., South, Press Brakes, Beyeler Mach-0935, Wysong Mach-0707, Wysong Mach-0750 hydraulic press brakes: on or about May 15, 2014, employees bending and forming metal parts were exposed to caught in-between amputation hazard where adequate point of operation guarding systems or methods were not provided. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). The Tella Tool & Mfg. Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 110535, citation number 01, item number 002 and was affirmed as a final order on 5/07/2012, with respect to a workplace located at 1015 N.
