29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: 1015 N. Ridge Ave. building, South, press brakes: On or about August 25, 2015, and on subsequent dates, operators bending and forming metal parts on the Beyeler Mach-0935 hydraulic press brake had their finger(s) and hand(s) exposed to caught-in hazards likely to result in amputation where point-of-operation guards and/or safeguarding methods were not provided. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). The Tella Tool & Mfg. Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 976914, citation number 01, item number 003 and was affirmed as a final order on July 11, 2014, with respect to a workplace located at 1015 N. Ridge Ave., Lombard, IL 60148. The Tella Tool & Mfg. Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 110535, citation number 01, item number 002 and was affirmed as a final order on May 7, 2012, with respect to a workplace located at 1015 N. Ridge Ave., Lombard, IL 60148.
