On the Zone 2 roof where shingles were applied, the employer did not provide a anchor for employees who were on the roof attached to a lifeline without an anchor thereby exposing employees to a fall hazard.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system. a. On the 3rd floor of Zone 9 where employees framed the floor, there was a top rail missing from the guardrail system thereby exposing employees to a fall hazard. b. On the 3rd floor of Zone 4 where employees framed the floor, there was a 20 inch opening in the guardrail thereby exposing employees to a fall hazard. The KCL Framing Company was previously cited for a violation of this Occupational Safety and Health standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 945435, citation number 1, item number 3 and was affirmed as a final order on 12-4-2013, with respect to a workplace located at 1231 Gabrielle Elaine Dr. Columbus, Ohio 43228..
