29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. On the 2nd floor of zone 7-3 where employees were conducting punch out and clean up during the construction of the building, a 5.25 inch by 64 inch hole was cut into the floor and not covered, thereby exposing employees to a trip hazard.
