Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section. a. On or about May 14, 2014 - at the above addressed jobsite, employees were performing roofing operations from a ladder jack scaffold in front of the building, approximately fifteen (15) feet seven (7) inches above the ground, that was not designed by a qualified person, thereby exposing the employees to a fall hazard.

29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end. a. On or about May 14, 2014 - at the above addressed jobsite, employees were performing roofing operations from a ladder jack scaffold that had the platform extending greater than eighteen (>18) inches past its supports, thereby exposing the employees to fall hazards of approximately fifteen (15) feet seven (7) inches to the ground.

On or about May 14, 2014 - at the above addressed jobsite, an employee performing roofing operations from a ladder jack scaffold in the back of the building, approximately fifteen (15) feet three (3) inches above the ground, was not protected by the use of a personal fall arrest system, thereby exposing the employee to fall hazards.
