29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches. a.) Barge Linda, Colonna's Shipyard, West Yard: On or about 05/12/14, two spud holes 32" wide and 8' deep were not provided with a guard or cover to protect employees from a potential fall of approximately 15 feet to the ground.
