29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program: a) Palmer Plastics - A Machine Operator feeding the conveyor of the new #6 Grinder was exposed to continuous noise at 154.7% of the permissible exposure limitl of 90 dBA or an eight-hour time weighted average of 93.1 dBA. This exposure was observed over a 473 minute sampling period. A zero exposure level was assumed for the 7 minutes not sampled. The employer did not conduct annual noise training for those employees exposed over the action level of 85 dBA observed on or about May 15, 2014. b) Palmer Plastics - A Machine Operator operating the vacuum hose for #2 Grinder was exposed to continuous noise at 111.6% of the permissible exposure limit of 90 dBA or an eight-hour time weighted average of 90.8 dBA. This exposure was observed over a 470 minute sampling period. A zero exposure level was assumed for the 10 minutes not sampled. The employer did not conduct annual noise training for those employees exposed over the action level of 85 dBA observed on or about May 15, 2014. c) Palmer Plastics - A Machine Operator operating the extruder was exposed to continuous noise at 79.3% of the permissible exposure limit of 90 dBA or an eight-hour time weighted average of 88.5 dBA. This exposure was observed over a 455 minute sampling period. A zero exposure level was assumed for the 25 minutes not sampled.The employer did not conduct annual noise training for those employees exposed over the action level of 85 dBA on or observed on or about May 15, 2014. No abatement documentation required.
