29 CFR  1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:   a) 2906 WILLIAM PENN HWY, SUITE 505 EASTON, PA 18045, in the Cumberland Grinder machine area and throughout the facility, on or about August 11, 2025 - the employer failed to ensure that the working surface in and around the plant was clean and orderly. Employees exposed to slip, trips, and fall due to excessive amounts of plastic dust on the walking working surface.   b)  2906 WILLIAM PENN HWY, SUITE 505 EASTON, PA 18045, on the Vecoplan Shredder Machine elevated platform, on or about August 11, 2025 - the employer failed to ensure that the working surface on an elevated platform was clean and orderly. Employees, feeding the Vecoplan Shredder Machine, were exposed to slip, trip, and fall hazard due to debris and extraneous material on the platform.   c) 2906 William Penn Hwy, Suite 505, Easton, PA 18045, in the baler area, on or about August 11, 2025 - the employer failed to ensure that the working surface in front of a bailer was clean and orderly. Employees were exposed to a trip hazard due to a steel plate covering a shallow section of missing concrete that was not secured to the surface.

29 CFR  1910.29(b)(10): The employer did not ensure that when guardrail systems are used at hoist areas, a removable guardrail section, consisting of a top rail and midrail, are placed across the access opening between guardrail sections when employees are not performing hoisting operations. The employer may use chains or gates instead of a removable guardrail section at hoist areas if the employer demonstrates the chains or gates provide a level of safety equivalent to guardrails:  a)  2906 WILLIAM PENN HWY, SUITE 505 EASTON, PA 18045, on or about August 11, 2025 - The employer utilized chains instead of a removable guardrail section at fork lift loading area. The employer did not demonstrate the chains provided a level of safety equivalent to guardrails. Exposing employees, operating a Cumberland Grinder machine, to a fall hazard from approximately 6 ft in height.     b)  2906 WILLIAM PENN HWY, SUITE 505 EASTON, PA 18045, on or about August 11, 2025 - The employer utilized chains instead of a removable guardrail section at fork lift loading area. The employer did not demonstrate the chains provided a level of safety equivalent to guardrails. Exposing employees accessing the Vecoplan Shredder Platform, to a fall hazard from approximately 6 ft in height.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) 2906 WILLIAM PENN HWY, SUITE 505 EASTON, PA 18045,  next to the Dynisco Extrusion pelletizer, on or about August 11, 2025 - electrical tape was used to attempt to fix a cut in the power cord to the machine control panel.  Electrical tape was not used or installed in accordance with instructions included in the listing or labeling, exposing employees to shock hazard.  b) 2906 WILLIAM PENN HWY, SUITE 505 EASTON, PA 18045,  next to the Dynisco Extrusion pelletizer, on or about August 11, 2025 - equipment was not used or installed in accordance with instructions included in the listing or labeling.  Employees exposed to shock and burns due to a 460 volt electrical panels having a broken panel box door lock.  Employer utilized a piece of rope to attempt to hold the panel door shut.

Employees exposed to electrical shock and burns due to electrical panels and disconnects were  obscured by garbage and debris.   b)2906 WILLIAM PENN HWY, SUITE 505 EASTON, PA 18045,  behind the PPK Grinder, on or about August 11, 2025 - Sufficient access and working space was not provided and maintained about all electric equipment. Employees exposed to electrical shock and burns due to electrical panels and disconnects were obscured by pallets and debris.
