On May 16, 2014, on the south side, front of the project, the scaffold platform was not fully planked, exposing an employee to a 10 feet 6-inch fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. On May 16, 2014, on the north side, rear of the project, an employee operating a hand grinder while standing on a walking working surface/platform without fall protection was exposed to a 14-foot fall hazard.
