OSH ACT of 1970 SECTION (5)(a)(1):  The employer did not furnish employment and  a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:  (a). In the warehouse the storage racks were not anchored exposing the employees in the area to being struck-by falling materials, on or about May 16, 2014.

29 CFR 1910.24(h):   Standard railings were not provided on the open sides of all exposed stairways and stair platforms:    (a) In the manufacturing production area the 5-foot stairway used to access the platforms to the #6, #7 and #8 Tyrine Hoppers did not have railings installed to provide safe access and egress, on or about May 16, 2014.

In the warehouse along the exit route located in the front of the building, the doorways along the exit path were not identified to show the exit or not an exit on or about May 16, 2014.
