29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     a)  Core Duct Machines.  The employer did not ensure that effective lockout/tagout procedures were developed, documented and utilized by operators when clearing mylar tape jams from the mandrel in the cutter area.  The employer's written procedures directed employees to lockout control circuit switches rather than electrical energy isolating devices on the machines. Employees did not lockout the electrical when clearing tape jams and were exposed to finger and hand amputation and laceration hazards from the cutter in the event of unexpected energization.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  a)  Core Duct Machine #2.  The employer did not ensure that all operators who cleared mylar tape jams in the knife cutter area affixed a lock to the disconnect for the knife cutter blades on the core duct machine.  Employees were exposed to amputation and laceration hazards.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The following Duct Assembly System (DAS) machines had Emergency Stop buttons on the decoilers/unwinders that were missing or malfunctioning, exposing employees to contusions if caught in rotating parts and unable to stop the machines.  a)    DAS Machine #2  b)    DAS Machine #5  c)    DAS Machine #6  There is no abatement documentation or certification required for this item.

29 CFR 1910.219(m)(1)(i): Standard conditions were not secured by the use of the following materials:  expanded metal, perforated or solid sheet metal, wire mesh on a frame of angle iron, or iron pipe securely fastened to floor or to frame of machine:  Core Flex Duct Machines.   Power transmission belt guards for differing mandrel diameters were not maintained in proper condition so that they could be securely fastened to the frame of the machines.  Employees were exposed to laceration hazards from wire used to improperly secure metal guards and struck-by hazards in the event of belt breakage on mandrels with improperly fitting guards.  a) Core Flex Duct Machine #1.  The belt guard for an 18" diameter mandrel was broken on one side where it was designed to be fastened to the machine.  Wire was used to temporarily fasten the guard to the machine during production.  Use of the 18" guard on the 16" mandrel created a gap between the belt and the guard.  b) Belt Guard Storage Area.  A belt guard for an 18" diameter mandrel that was hanging on the rack was broken on both sides where it was designed to be fastened to the machine, and not available to employees for use.   c) Belt Guard Storage Area.  A belt guard for a 16" diameter mandrel that was hanging on the rack was broken on one side where it was designed to be fastened to the machine, and not available to employees for use.  Abatement certification is required for this item in accordance with 29 CFR 1910.219.
