29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:   On or about May 22, 2014, an employee entered a hot oven on the Core Machine #2 line without any head protection to protect him from a thermal burn.  The oven was operating at 200 degrees Fahrenheit.   Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations:    On or about May 22, 2014, an employee entered a hot oven on the Core Machine #2 line in order to adjust the tension on the Mylar web.  The employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations to remove hazardous conditions including, but not limited to, locking out the oven and allowing it to cool.  The oven was operating at 200 degrees Fahrenheit.     Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    On or about June 11, 2014, the point of operation on the Sibley Drill Press, Model E-25, was not guarded.  The operator was exposed to the spinning drill bit during the operating cycle while placing and removing parts near the point of operation.
