On or about May 20, 2014 - at the above addressed jobsite, the employer did not initiate and maintain an accident prevention program to comply with the regulations of this part, which included but were not limited to, employee protection when working at heights.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about May 20, 2014 - at the above addressed jobsite, employees conducting roofing operations from a residential roof that measured eleven (11) feet one (1) inch, were not protected from falls, thereby exposing the employees to fall hazards. The AGG CONSTRUCTION INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10) which was contained in OSHA inspection number 923150, citation number 1, item number 1 and was affirmed as a final order on August 20, 2013, with respect to a workplace located at 707 Germania, Ottawa, IL 61350.
