29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about June 4, 2026, at the above addressed jobsite, employees using pneumatic nail guns to install roof shingles were not provided with eye protection, thereby exposing the employees to hazards associated with eye injuries.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a.  On or about June 4, 2026, at the above addressed jobsite, employees wearing personal fall arrest systems while performing residential roofing operations were not attached to the lanyards and ropes by the D-ring in the center of the employees' backs near shoulder level, thereby exposing the employees to fall hazards.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about June 4, 2026, at the above addressed jobsite, employees performing residential roofing operations had been provided fall protection training; however, the employer failed to verify the training by preparing a written certification of training record.
