29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 5/16/14, at the above addressed site, employees were exposed to a fall hazard while installing roof trusses next to a stairway opening on the 2nd floor with no guardrails.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On or about 5/16/14, at the above addressed site, the employer did not provide adequate fall prevention training to       employees exposed to fall hazards while installing sheathing on a residential roof.   b) On or about 5/16/14, at the above addressed site, the employer did not provide adequate fall prevention training to       employees exposed to fall hazards while installing trusses from the 2nd level of the town house.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  On or about 5/16/14, at the above addressed site, employees were using a stairway with four risers approximately five feet in height to access the 2nd floor, whereas the stairway was not equipped with a stairrail system.
