29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) On or about 6/5/14, at the above addressed site, an employee was exposed to a struck-by hazard while using a nailgun to install hangers without wearing eye protection.  b) On or about 6/5/14, at the above addressed site, an employee was exposed to a struck-by hazard while using a hammer to remove nails from the trusses of a commercial property without wearing eye protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 6/5/14, at the above addressed site, employees were exposed to a fall hazard while using a ladder to access the top level, which did not extend 3' or more above the upper landing.   b) On or about 6/5/14, at the above addressed site, employees were exposed to a fall hazard while using a ladder to access the roof, which did not extend 3' or more above the upper landing.
