29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  At this location, employees performed brick masonry work from the top of a scaffold at a height greater than forty feet above the ground.  The scaffold poles did not bear on base plates or mudsills exposing employees to a fall hazard to concrete.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  At this  worksite, employees worked on the top level of a scaffold at a height greater than forty feet.  The scaffold was not equipped with adequate fall protection such as a guardrail system.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks:  At this work site, employees operated a powered industrial truck to move mason bricks and the seatbelt was not functional.
