29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location: Jobsite  Employees were exposed to fall hazard of greater than 6 feet to the ground below, while working where fall protection had not been used or provided.  Instance 1) Employees were accessing a carpenters scaffold on the second floor through a wall opening, where no fall protection was used or provided. No guardrails were installed, and no personal fall arrest system was used.  Instance 2) An employee was exposed to a fall hazard greater than 6 feet at an unguarded first floor wall opening, at the back entrance of the building under construction.  Instance 3) An employee was exposed to a fall greater than 6 feet to the basement below, where the floor opening was not completely covered.
