Winesburg Builders did not provide certification on each of the cited items: a. On the roof of the hotel where sheets of metal were placed over the plywood decking, the employees were using one anchor rated at 5000 pounds for two employees. b. In the south stairwell the employer did not install handrails. c. In the north stairwell the employer did not install a stair rail.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On the roof where employees were unloading sheets of metal to be installed over a wood roof deck, employees were observed working without fall protection thereby exposing the employees to a fall hazard.
