29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: (a) On or about May 19, 2014, and at times prior, Tee Pee Corporation, as exposing and controlling employer, did not ensure that its employees and employees of Naco's Crew Construction, Inc., subcontractor, were protected from trip and fall hazards from fourteen holes in the floor along the primary walkway in a building under construction. The holes were 12 inches deep and 24 inches in diameter. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: (a) On or about May 19, 2014, and at times prior, Tee Pee Corporation, as exposing and controlling employer, did not ensure that its employees and the employees of Naco's Crew Construction, Inc., subcontractor, were protected from falls while walking up and down the north stairwell of a four story building, which did not have handrails. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
