29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: (a) On or about April 27, 2016, Tee Pee Corporation, as the controlling employer, did not ensure that scaffolds and scaffold components were inspected for visible defects by a competent person before each work shift. A seven-tier scaffold in use by employees of Durango Plastering & Drywall, a subcontractor, was not inspected for visible defects before each shift. Scaffold did not have properly installed guardrails, exposing employees to falls in excess of 50 feet. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway: (a) On or about April 27, 2016, Tee Pee Corporation, as the controlling employer, did not ensure that each employee on a walkway within a scaffold was protected by a guardrail system with a minimum 200 pound toprail capacity. A seven-tier scaffold in use by Durango Plastering & Drywall, a subcontractor, had spliced guardrails joined by a single strand of tie wire, exposing employees to falls in excess of 50 feet. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
