OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of a fall from elevation: a) Fourth floor, STOR-ALL Storage located at 2515 Cumberland Parkway, Atlanta, GA 30339: Employees were exposed to a falls of approximately 11-feet to the concrete surface below while engaged in welding activities standing on the mid and top guardrails of a scissor lift. The employees were using a Sky-Jack (SJ111-3219) Scissor Lift and failed to keep both feet firmly positioned on the platform floor at all times.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: (a) Roof, STOR-ALL STORAGE located at 2515 Cumberland Parkway, Atlanta, GA 30339: CSHO observed employees exposed to fall hazards of approximately 45-feet to the ground level while installing metal roof decking as no fall protection system was provided. (b) Fourth, floor, STOR-ALL STORAGE located at 2515 Cumberland Parkway, Atlanta, GA 30339: CSHO observed employees exposed to falls of approximately 32-feet while working in close proximity to an unprotected edge as the guardrails were not replaced after material had been hoisted to the fourth floor.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Fourth floor, STOR-ALL STORAGE located at 2515 Cumberland Parkway, Atlanta, GA 30339. Employees were exposed to falls of approximately 13-feet to the concrete surface below when the Werner (Model-D1116-2) 16 ft. Aluminum Ladder provided did not extend at least 3-feet past the roof to allow employees safe access on and off the working surface. The ladder provided was not extended or secured to the structure to prevent accidental movement during use.
