29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 5/22/14 at a residential new build site located at 42 Newberry Lane, Lancaster, New York; employees were using a ladder jack scaffold while installing shingles on the roof. There was no personal fall arrest system in use.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about 5/22/14 at a residential new build site located at 42 Newberry Lane, Lancaster, New York; employees were using extension ladders to access the ladder jack scaffold and the roof while installing shingles. The ladders only extended 1 foot above the scaffold platform.    NO ABATEMENT CERTIFICATION REQUIRED

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 5/22/14 at a residential new build site located at 42 Newberry Lane, Lancaster, New York; employees were installing shingles on a new roof with a 7 in 12 pitch and 18 foot eave height. There was no fall protection in use.    NO ABATEMENT CERTIFICATION REQUIRED    Irish Pride Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 543545, citation number 1, item number 1 and was affirmed as a final order on 8/20/12, with respect to a workplace located at 10 Wetherby Way, Lancaster, New York.
