(a) Throughout facility: The machine specific lockout tagout procedures for machines that had more than one power source, and were hard wired in, such as, but not limited to, the Verson Press F165, the Niagara Press F185, and Bliss Press, were not descriptive of the type of machine, the location to lock and tag out the piece of equipment and steps to reenergize machine following maintenance completion.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Maintenance Shop Area: Employees were exposed to the Radial Mill Drill Press which did not have an adjustable guard to cover the unused portion of the bit and chuck above the material being worked. (b) Hitch Feed Area: Employees were exposed to unguarded machinery and sharp metal as the Steel Uncoilers for the Verson F165, and the Niagara F185 presses did not have a barrier guard.
