29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:     (a) Machine Shop; The employer did not ensure that the Toyota Forklift model # 42-6FGCU20, serial #67584-8 was equipped with a safety belt. Employees were exposed to crush-by hazards in the event of a tip over.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:       (a) Machine Shop; Fabrication Shop; Employees performed maintenance (change blades) on equipment such as, but not limited to, horizontal saw without ensuring that all energy sources were de-energized and isolated. This equipment utilizes electric and pneumatic energy sources.  Without energy control procedures, employees could incur serious injuries as a result of unexpected energizing, start up or release of stored energy.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by the rotating chuck:      a) Machine Shop area: The employer failed to ensure that a OOYA Radial Drill model YMR-915 #OA512387 used by the employees was equipped with a chuck guard.  Employees were exposed to laceration hazards.     b) Machine Shop area: The employer failed to ensure that a Allen Drill model 1952 #28651 used by the employees was equipped with a chuck guard.  Employees were exposed to laceration hazards.    c) Machine Shop area: The employer failed to ensure that a Ron Smith Table Drill used by the employees was equipped with a chuck guard.  Employees were exposed to laceration hazards.
