(a) Parking Deck, Hilton Homewood Suite Hotel located at 915 Crestline Parkway, Sandy Springs, GA 30328: Employees suffered injuries while installing Hollowcore Concrete Slabs on a Diversakore steel frame as the shoring supports used were not capable of supporting a load of approximately 67,000 pounds. During the installation of the concrete slabs it was observed that shoring was falling out of place as the slabs were set and work was not stopped when these failures occurred.

The shoring contractor was not on site the day the slabs were installed.
