29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     On April 15, 2014 employees were not protected from a fall while roofing a residential property, exposing employees to a fall hazards of 13 feet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person.     On April 15, 2014 an employees was exposed to a fall as the anchor was not installed under the supervision of a qualified person and not capable of supporting 5,000 pounds.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.    On or about  April 15, 2014 employees were not provided with fall protection training and unable to recognize or minimize the hazards associated with a fall.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
