29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about May 22, 2014, at the worksite located at Meadowood, Blvd., Twinsburg Ohio, the employer failed to instruct employees on how to recognize unsafe conditions while working in front of, behind, and to the side of concrete trucks, while actively pouring concrete, and to ensure that employees are aware of the need to remain in the clear line of sight of the vehicle operations, and to take steps necessary to ensure that all employees are in safe locations prior to authorizing the movement of any vehicle.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical. On or about May 22, 2014 at the worksite located at Meadowood, Blvd., Twinsburg Ohio, the employer did not maintain a copy of the safety data sheet for the Seal Tight 1600-White concrete curing compound which employees spray on the concrete, after it is poured, to help to seal it and cure.
