On or about May 22, 2014, at the worksite located at Meadowood, Blvd., Twinsburg Ohio, the employer failed to instruct employees on how to recognize unsafe conditions while working in front of, behind, and to the side of concrete trucks, while actively pouring concrete, and to ensure that employees are aware of the need to remain in the clear line of sight of the vehicle operations, and to take steps necessary to ensure that all employees are in safe locations prior to authorizing the movement of any vehicle.
