29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level: (a) On or about May 19, 2014 the inspection revealed that employees were potentially exposed to serious injury from a fall to a lower level due to the lack of a midrail. Located on the second story mezzanine above the dry room of the seed extractor, at the Bend Pine Administration Site in Bend, Oregon.

29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: (a) On or about May 19, 2014 the inspection revealed that employees were potentially exposed to serious injury from falls to a lower level due to a fixed ladder that did not have a swing gate or offset at the top. The ladder was located outside the loading dock by cooler door C at the Bend Pine Administration Site. The Forest Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 106753,notice number 2, item number 1 and was affirmed as a final order on November 25, 2011, with respect to a workplace located at Halfway, Oregon.

The Forest Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 312391188, notice number 1, item number 1 and was affirmed as a final order on February 23, 2010, with respect to a workplace located at Wallowa Whitman National Forest.

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects: (a) On or about May 19, 2014 the inspection revealed that employees were potentially exposed to serious injury from a Lincoln Idealarc Welder that did not have covers over the lead terminals. The welder was located in the Central Oregon Combined Off Highway Vehicle Operations shop in the west corner welding area, at 63095 Deschutes Market Road in Bend, Oregon. The Forest Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 898290, notice number 1, item number 5 and was affirmed as a final order on May 10, 2013, with respect to a workplace located at Oakridge, Oregon.
