29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: Cooler doors C: On or about May 19th, 2014 and times prior thereto, the Hercules cooler door would not stop and reverse itself if something or someone got between it. This exposed employees to potential caught between hazard if they walked between the doors during operation.

29 CFR 1910.213(b)(3): Provisions were not made to prevent woodworking machine(s) from automatically restarting upon restoration of power after power failure. (a) On or about May 19th, 2014 and times prior thereto, the Rockwell table saw did not have a device to prevent automatic restart after a power failure. This exposed employees to potential amputation or struck-by hazards if table saw restarted unexpectedly. The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.213(b)(3), which was contained in OSHA inspection number 898288, notice number 1, item number 1 and was affirmed as a final order on July 6/2013, with respect to a workplace located at Rigdon Flatcreek, OR.

This exposed employees to potential struck-by hazards if the wheel were to break during use.
