29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:  Cooler doors C:  On or about May 19th, 2014 and times prior thereto, the Hercules cooler door would not stop and reverse itself if something or someone got between it.  This exposed employees to potential caught between hazard if they walked between the doors during operation.

29 CFR 1960.25(c): Sufficient unannounced inspections and unannounced follow-up inspection were not conducted by the agency to ensure the identification and abatement of hazardous conditions.     (a) Although the unit had established an active annual facility safety inspection program, as required by 29 CFR 1960.25, the inspection process failed to identify several hazardous conditions that had previously been identified on other forests throughout the Region.

29 CFR 1910.213(b)(3): Provisions were not made to prevent woodworking machine(s) from automatically restarting upon restoration of power after power failure.  (a)  On or about May 19th, 2014 and times prior thereto, the Rockwell table saw did not have a device to prevent automatic restart after a power failure.  This exposed employees to potential amputation or struck-by hazards if table saw restarted unexpectedly.   The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.213(b)(3), which was contained in OSHA inspection number 898288, notice number 1, item number 1 and was affirmed as a final order on July 6/2013, with respect to a workplace located at Rigdon Flatcreek, OR.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Central Oregon Combined Off Highway Vehicle Operations shop:  On or about May 19th, and times prior thereto, the Jet bench grinder measured 1/2 inches from wheel to tongue guard.  This exposed employees to potential struck-by hazards if the wheel were to break during use.  The Forest Service was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 312393465, notice number 2, item number 6 and was affirmed as a final order on 08/18/2011, with respect to a workplace located at Lookout Mt. Ranger District.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  (a)  Seed extractor main bay:  On or about May 19, 2014 and times prior thereto there was an air nozzle that would not reduce to below 30 psi.  This exposed employees to potential air embolism or laceration hazard if it contacted their skin.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.242(b), which was contained in OSHA inspection number 903923, notice number 1, item number 5 and was affirmed as a final order on 07/01/2013, with respect to a workplace located at Tiller Ranger District in Roseburg, Oregon.

This exposed employees to potential shock hazard from incidental contact with live electrical parts.   The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(b)(2), which was contained in OSHA inspection number 312391923, notice number 2, item number 4 and was affirmed as a final order on 07/14/2010, with respect to a workplace located at Bly, Oregon.
