29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about May 28, 2014 and at times prior thereto, the employer did not ensure that unused openings were effectively closed or covered exposing employees to electrical shock and fire hazards: a) In the compressor room the 480 volt square D east and west panel boxes had opened holes in the front panel and on the bottom of the panels; b) In the box storage room a 6 amp, 240 volt 3 phase panel box had opened holes in the bottom; c) In the boiler room the electrical circuit breaker panel had two opened holes; and d) In the small deboning entry room the circuit breaker panel box had an opened hole and the panel box had an opened hole in the bottom.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: On or about May 28, 2014 and at times prior thereto, the employer did not ensure that employees were not exposed to electrical and fire hazards where approved covers were not provided: a) In the compressor room, the 480 volt raceway cover was not attached and opened exposing the internal wiring, and two Mercoid controls did not have covers; b) Outside of the small kill room, the Leeson water pump was plugged into a two receptacle outlet box that did not have a cover; and c) In the small kill deboning room against the back wall behind the conveyer, a 120 volt two receptacle outlet box did not have a cover.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about May 28, 2014 and at times prior thereto, in the large deboning room 7041B, the employer did not ensure that the model 55 meat band saw that was being operated was adjusted to the height of the material being cut. The blade guard was locked in the upright highest point with approximately 14 inches of rotating blade exposing the employees to amputation hazards. The Beltex Corporation (Frontier Meats) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 661358, citation number 1, item number 2 and was affirmed as a final order on 04/04/13, with a final abatement date of 04/09/13, with respect to a workplace located at 3801 N. On or about May 28, 2014 and at times prior thereto, the employer did not ensure that all sprockets and chains in the plant were guarded to protect the employees from caught in and amputation hazards: a) Outside the small kill room employees operated a conveyer belt in the Boar hide collection area and the sprocket and chain was not fully guarded; and b) In the small kill processing area employees operated a conveyer belt and the sprockets and chain underneath the conveyor were not guarded.
