29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:    Representative noise monitoring was not conducted for the employee with the following exposure:    a) Warehouse, abrasive blasting booth - An abrasive blaster was exposed to continuous noise levels in excess of the allowable 8 hour time-weighted average sound level (Permissible Level). The employees dosimeter dBA level was 1156% of the permissible level (90 dBA) and was equivalent to approximately 113.0 dBA. The sampling was performed for 410 minutes during one shift on 6/4/14. Zero exposure was assumed for the unsampled period of time.

29 CFR 1910.95(i)(2)(i): The employer did not ensure that hearing protectors are worn by an employee who is required by 29 CFR 1910.95(b)(1) to wear personal protective equipment:    Hearing protection was not worn by the employee with the following exposure:    a)  Warehouse, abrasive blasting booth - An abrasive blaster was exposed to continuous noise levels in excess of the allowable 8 hour time-weighted average sound level (Permissible Level). The employees dosimeter dBA level was 1156% of the permissible level (90 dBA) and was equivalent to approximately 113.0 dBA. The sampling was performed for 410 minutes during one shift on 6/4/14. Zero exposure was assumed for the unsampled period of time.      NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    a) Warehouse - Employees who wore tight fitting respirators including, but not limited to 3M half mask and 3M full face air purifying respirators were not medically evaluated for respirator use. On or about 5/22/14.    Milspray, LLC. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, 29 CFR 1910.134(e)(1), which was contained in OSHA Inspection Number 315060731, Citation Number 1, Item Number 3, and was affirmed as a final order on 3/29/2011, with respect to a workplace located at 845 Towbin Avenue, Lakewood, New Jersey 08701.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  Warehouse - Training in the hazards of chemicals in the workplace was not provided for all employees who were exposed to chemicals including, but not limited to paints and abrasive blasting materials.
