At the facility located at 444 Hebron Road, Heath, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2013 a description of work-related injuries and illnesses.  On or about December 9, 2013, an employee had a  contact dermatitis, headache and cough, but there was no description of what caused the injury and/or what the employee was doing when the injury occurred.  b.  At the facility located at 444 Hebron Road, Heath, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2014 a description of work-related injuries and illnesses.  On or about March 24, 2014, an employee had a right elbow/forearm tendonitis, but there was no description of what caused the injury and/or what the employee was doing when the injury occurred.  c.  At the facility located at 444 Hebron Road, Heath, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2014 a description of work-related injuries and illnesses.  On or about April 11, 2014, an employee had a left ring finger laceration but there was no description of what caused the injury and/or what the employee was doing when the injury occurred.
