Toledo Metal Spinning Co. located at 1819 Clinton St. Toledo, OH 43607; On or about May 23, 2014, the employer did not ensure the Acrotech Diacro rolling machine was bolted to the floor. On May 23, 2014, employees were attempting to adjust the tightness of the metal cylinder and an employee was crushed by the Diacro rolling machine.
