Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a) Workshop Floor - On or about August 21, 2014, employees were exposed to overhead struck-by hazards when loading steel on a truck without wearing hard hats.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) North Shop - On or about May 30, 2014, specific procedures were not developed, documented and utilized for "The Niagara 1/2"/10ft shear" which has 2 sources of energy consisting of hydraulic and electric energy for the maintenance employee who performs preventative maintenance on the equipment.    b) North Shop - On or about May 30, 2014, specific procedures were not developed, documented and utilized for the "Roundo PS 360 plate rolls" which has 2 sources of energy consisting of hydraulic and electric energy for the maintenance employee who performs preventative maintenance on the equipment.    c) North Shop - On or about May 30, 2014, specific procedures were not developed, documented and utilized for the "Roundo PS 260 plate rolls" which has 2 sources of energy consisting of hydraulic and electric energy for the maintenance employee who performs preventative maintenance on the equipment.    d) North Shop - On or about May 30, 2014, specific procedures were not developed, documented and utilized for the "Roundo R4 angle rolls" which has 2 sources of energy consisting of hydraulic and electric energy for the maintenance employee who performs preventative maintenance on the equipment.    e) South Shop - On or about May 30, 2014, specific procedures were not developed, documented and utilized for the "Accurpress 400 ton press brake" which has 2 sources of energy consisting of hydraulic and electric energy for the maintenance employee who performs preventative maintenance on the equipment.    f) South Shop - On or about May 30, 2014, specific procedures were not developed, documented and utilized for the "Niagara 200 ton press brake" which has 2 sources of energy consisting of hydraulic and electric energy for the maintenance employee who performs preventative maintenance on the equipment.    g) South Shop - On or about May 30, 2014, specific procedures were not developed, documented and utilized for the "Akyapak 5/8"/10 plate rolls" which has 2 sources of energy consisting of hydraulic and electric energy for the maintenance employee who performs preventative maintenance on the equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Workshop Floor - On or prior to August 21, 2014, employees were exposed to caught-in hazards due to operating an Eagle Bending Machines (Model CPS 20) roll-bending machine without adequate machine guarding.    b) Workshop Floor - On or prior to August 21, 2014, employees were exposed to caught-in hazards due to operating a Peddinghaus (Model 210 SUPER 20, Nr. 4412457773001) Ironworker machine without adequate machine guarding.     c) Workshop Floor - On or prior to August 21, 2014, employees were exposed to caught-in hazards due to operating an Original Peddinghaus (Type: 210/13, Mach. No. 638) the smaller Ironworker machine without adequate machine guarding.
