29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   (a) On or about May 20th, 2014, and at certain times before, employees operating the Line 27 and Line 28 extruding machines were exposed to finger and hand amputation hazards in that they were allowed or required to reach underneath interlocked barrier guards to clear product from the output side of the conveyor-fed shearing mechanisms.  These actions were done routinely without the machines being locked out, although the process of clearing the jams did require employees to reach into the (shearing) point of operation areas on the two machines.    Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:   (a) On or about May 20, 2014, and at certain times before, employees operating the Line 27 and Line 28 extruding machines were exposed to finger and possible hand amputation hazards due to inadequate guarding systems on the output sides of the shearing devices near the end of each Line.  The guards (interlocked hinged sheets of transparent vinyl or plastic) on the output sides of the shears allowed excessive clearances below their lower edge such that an employee could reach underneath the guards (in their "closed" positions) and easily reach the shearing points.  On or about May 16th, 2014, an employee working on the Line 27 extruder machine suffered the amputation of their right ring finger at the first knuckle, and the tip of their right little finger.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
