(a) Corner of East Prospect and Cape Horn Road, York, Pennsylvania - An employee walked through an unprotected trench that was 9 1/2 feet deep, 15 feet wide and 50 feet long to access a 5 foot X 5 foot concrete storm drain box where he was directed to position a construction laser on or about May 27, 2014. No abatement certification or documentation required.
