OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from crushing and/or struck by hazards: (a) Facility, 4 Pennwest Way, Emlenton, PA: On or about 05/30/14 the employer did not conduct and record inspections of the hoist, sheaves, hooks, chains, ropes, wheels and other lifting devices on the A4 underhung crane to ensure no deficiencies existed.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees by exposing them to falling loads and crushing hazards: (a) Jobsite, 4 Pennwest Way, Emlenton, PA: On or about 06/09/14 the employer did not ensure that an appropriate under the hook lifting device was used in the ceiling build up department. An under the hook lifting device being used to lift loads had not been designed, built or approved by a qualified person exposing employees to hazards associated with falling loads. On 05/14/14 several chains on the A4 under the hook lifting device used to secure a completed ceiling unit that was approximately 30 feet in length broke from their weld causing the unit to fall onto the work floor below.
