29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:   a) On May 28, 2014 and at times prior thereto; second floor; employees were using unguarded energized outlets either plugged in directly or by portable extension cords to power electrical such as but not limited to saws.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.  a) On May 28, 2014 and at times prior thereto; Third floor; employees were using portable electrical equipment, such as but not limited to circular saws, that were plugged into outlets either directly or by portable extension cords which were not equipped with a ground fault circuit interrupter.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On May 28, 2014 and at times prior thereto; roof area; employee was working on the roof removing water that had collected in the blue tarp without a personal fall arrest system in place to prevent him from falling 35 feet to ground level.    b) On May 28, 2014 and at times prior thereto; third floor; employees framing the residential home were observed working near unguarded floor openings exposing them to a fall hazard to the lower levels; 10-20 foot fall.     c) On May 28, 2014 and at times prior thereto; third floor; employees framing the residential home were working around unguarded wall openings exposing them to 10-20 foot fall to the lower levels.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed   a) On May 28, 2014 and at times prior thereto; second floor; the ladder used to access the work area was set flush to the working platform exposing them to a fall hazard of 12 feet to ground level while accessing or stepping off the ladder to the platform.
