29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 1717 DAUPHIN AVENUE, WYOMISSING, PA - Employee(s) performed residential roofing work on a multi level home, and the employees were not protected by any kind of fall protection system, thereby exposing the employee(s) to a fall hazard that ranged from nine (9) to twenty (20) feet to the ground below, on or about May 29, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) 1717 DAUPHIN AVENUE, WYOMISSING PA, FRONT OF HOUSE - Employees used a Werner 24 foot extension ladder to access the roof, and the top of the ladder did not extend a minimum of three (3) feet above the landing surface, thereby exposing employees to fall hazards of approximately nine (9) feet to the ground below, on or about May 29, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
