29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  1645 East Thistle Drive, Wyomissing Pennsylvania - Employees worked on a roof without fall protection exposed to a fall greater than 6 feet.  The employer did not ensure that all employees exposed to fall hazards were provided with conventional fall protection methods, on or about June 20, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  (a)  1645 East Thistle Drive, Wyomissing Pennsylvania -  Employees worked on a roof more than 6 feet above the lower level.  The employer did not ensure that employees were provided with training on the proper use of a harness, on or about June 20, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
