29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) 1889 Shady Oak Road Mount Joy, PA  - Employees did not don safety glasses while using pneumatic nail guns to attach shingles to the roof of a townhome building and were exposed to flying debris and ricochet nails, on or about July 12, 2018.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)  1883 Shady Oak Drive Mount Joy, PA - Employees were observed performing roofing activity on a steep residential roof, with a 10 in 12 pitch without the benefit of a complete fall protection system, thereby exposing employees to a fall of 20 feet to the ground below, on or about July 12, 2018.    WGM Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 982317, citation number 1, item number 1 and was affirmed as a final order on Friday October 17, 2014 with respect to a workplace located at 1645 East Thistle Drive Wyomissing, PA 19610.    WGM Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 979092, citation number 1, item number 1 and was affirmed as a final order on July 14, 2014, with respect to a workplace located at 1717 Dauphin Avenue, Wyomissing PA 19610.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
