Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to a fire and explosion hazard operating a RZ 4000 Coffee Bean Roaster: (a) On or about June 3, 2014, in the RZ 4000 Roasting Area control room, employees operating the roaster were exposed to fire and explosive debris when the employees were not adequately trained to implement emergency procedures for controlling an exothermic reaction that occurred during the coffee roasting process.
