29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support platforms while the entire platform was not attached to the forks: a) Wood Mill Drive, East Windsor, NJ: On or about 06/03/2014, a MS Construction company, employee was noted working from the raised forks of a GEHL Dynalift telescoping boom, rough terrain fork lift (RTFL), using a plywood job-build three sided box as his walking working platform. The platform was not attached in any way to the machine.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. a) Wood Mill Drive, East Windsor, NJ: On or about 06/03/2014, a MS Construction company, employee was noted working from the raised forks of a GEHL Dynalift telescoping boom, rough terrain fork lift (RTFL), using a plywood job-built three sided box as his walking working platform. The worker was not protected by guardrails, fall restraint, PFAS or any other form of fall protection and was thus exposed to a fall hazard of 20 feet from the working surface to the ground.
