MS Construction, Intersection of Central Ave. and School Street, Oak Hill WV:  On or about 08/25/15, employees performing roofing work on a 4/12 sloped roof were not protected from falling 18' to the ground.    ABATEMENT DOCUMENTATION IS REQUIRED.

MS Construction, Intersection of Central Ave and School Street, Oak Hill WV:  On or about 08/25/15, employees were using a Werner extension ladder to access the roof that was not properly extended at least 3 feet above the landing surface.    ABATEMENT CERTIFICATION IS REQUIRED.
