On or about June 3, 2014 - at the above addressed jobsite, the manufacturer's specifications, recommendations, and limitations, and manufacturer's approval to deviate from the specifications, recommendations, and limitations were not available in a written form at the jobsite during construction of the protective systems.

29 CFR 1926.652(e)(1)(ii): Support systems shall be installed and removed in a manner that protects employees from cave-ins, structural collapses, or from being struck by members of the support system. a. On or about June 3, 2014 - at the above addressed jobsite, an employee was installing the speed shore support system from the inside of an eight (8) foot eight (8) inch deep trench, thereby exposing the employee to the hazard associated with cave-ins.
