29 CFR 1926.502(i)(3): All covers shall be secured when installed so as to prevent accidental displacement by the wind, equipment, or employees. a. On or about April 28, 2015 - at the above addressed jobsite, throughout the entire project (floors 4 - 15) the plywood pieces used to cover the floor holes were not secured as to prevent accidental displacement, thereby exposing employees and subcontractor employees to trip hazards.
